Congratulations on your decision to take control of your health and wellness! To get you started on your journey, please complete and submit the enrollment form below. After you submit your enrollment form, you will receive an invoice email within 1 to 3 business days. You can pay the invoice directly from the email with your choice of payment options (credit card and Apple Pay, Bank ACH payment, Paypal, Venmo, and check). Once your payment is processed, you will receive an email containing important client forms that you will need to complete, sign, and return to us in order to participate in the program. After we receive your completed and signed client forms, that's it! Your journey can begin, and we are excited to partner with you to achieve your best self!
